SERVICES
Dr. Yuan utilizes an personalized and integrative approach to evaluating and addressing mental health issues. Psychiatric Consultations are 90 minutes and include a comprehensive assessment and options for treatment. Follow up visits with medication management are 25 minutes. As part of a Personalized and Integrative approach, pharmacogenetic testing, nutritional, and other cutting edge studies are available.
WHAT TO EXPECT DURING AN INITIAL CONSULTATION
An initial visit is a comprehensive consultation to determine how to best assist individuals with their current goals in therapy. During this evaluation, medical, psychological, and social histories will be reviewed in order to develop an effective treatment plan.
Once you Request an initial consultation, please complete the NEW CLIENT FORMS before your appointment & bring them with you so we can focus on your current concerns and treatment plan at your consultation.
* Please arrive with plenty of time
If you are unable to complete the paperwork before your visit, please arrive 30 minutes ahead of your appointment to complete them at the office. The information on the forms are important for your care and to establish you as a client in the practice.
* What to bring to your Initial Consultation
Please bring the NEW CLIENT FORMS and the current medications you are taking. Please bring any copies of past medical or psychiatric records, laboratory or other test results. Please have contact information for any current or past physicians or health care providers available.
When you arrive, please push the switch with Dr. Yuan's name to the left and relax in the waiting room until you are called in for your appointment.
MEDICATION POLICY
Medications can be prescribed only after the initial evaluation has been completed. Established patients will be given enough medication refills until the next office visit, so refills by phone are not necessary. This is to limit medication errors and to protect your safety. If you have to reschedule an appointment and you are low on medication, you can request a refill to ensure you have enough medication until the re-scheduled visit.
INSURANCE
Dr. Yuan does not participate in any insurance panels for this outpatient practice. This is to ensure clients receive high quality care tailored to their needs rather than insurance company demands. Clients interested in utilizing insurance will pay out-of-pocket and be provided with an invoice (superbill) that can be submitted for reimbursement. Please check with your carrier for out-of-network mental health service benefits or pre-approval requirements.
FEES
Payment for services is collected at the time of appointment. Payments can be made in cash or check.
$350 for 80 minute initial consultation
$300 for 50 minute therapy sessions, with or without medications
$200 for 25 minute medication follow-up visits, nutreval or genecept test consultations
CANCELLATION AND NO-SHOW POLICIES
Consistent follow-up appointments are important for safe and effective care. A 24 hour notice is required to cancel or change your appointment to avoid late cancellation fees. Please call and leave a message to cancel. Dr. Yuan has limited availability - early notification allows other patients to utilize that time. Cancellations made less than 24 hours will be billed in full for the missed appointment. If you are more than 10-15 minutes late to a 25 minute follow up appointment, you will be asked to reschedule for another day in order to provide safe and effective care for you and subsequent clients scheduled on that day.
